For the fifth year running, we have been looking at themes emerging from prevention of future deaths (PFD) reports issued by coroners in connection with the provision of health and social care.
This year's review covers themes identified from over 300 PFD reports issued over the course of 2025 in relation to acute hospital care, mental health, social care and ambulance services, including how these compare with previous years we have looked at.
