NHS England’s Chief Executive Simon Stevens’s recent NHS Five Year Forward View has provided further ammunition in the battle to drive closer integration between primary and secondary care, and social care and community services.

DAC Beachcroft has gauged the opinion of senior figures across the health and social care spectrum, who are engaged in the complex work of forming new and improved ways of delivering health and wellbeing.

The resulting report seeks to illustrate some of the drivers of health and social care integration, as well as identify the potential inhibitors or risks that may threaten joined-up working.
